The Disability Champions Project initiated is a three pronged mentoring project designed to support young people with disabilities as they transition from K-12 schooling to post-secondary settings including higher education, work, and community. A specific focus of the project is building community relationships based on the development of mentoring networks and an inclusive approach that addresses multiple marginalizing identities including disability, race, language, orientation, religion and gender as they affect experience.

The project has been developed collaboratively among individuals with disabilities, K-12 educators, community service, faith-based, family and university stakeholders.

1. Video Series:  In My Own Voice focused on stories of transition and mentoring networks that are multifaceted and shared to promote and inspire self-determination of outcomes among young people with disabilities. 
2. Mentoring Program F2F: includes the preparation of near peer mentors(young adults 18-30 with disabilities) and pairings with young people with disabilities ages 14-21 who are in the process of navigating the transition process. This component, in development, will include mentor preparation and community engagement sessions to support young people in the building of community, goal setting and pursuit of self-determined outcomes. 
3. Family Focus a particular focus on family information, support, advocacy and guidance. The ultimate goal is to develop a community of individuals who are connected through meaningful relationships and continuity.
