Face to Face Connecting with Schools, Families, Individuals
Mentor Champion Selection and Preparation
The mentor champions for the face to face connection will complete an application, interview and screening process including background checks. They will complete a 9-hour preparation program and participate in mentor champion development meetings each month.
Disability Champions Mentor Application
Mentor Champion Matching
Potential student champions will complete a program application to share their interests, goals, details of their location and disability in order for our program network to find an appropriate mentor match.
Mentor Network Support Team
Mentor champions and student champions will connect and engage with and through a mentoring network of professionals, families, educators and community members (network champions) with expertise to support relationship development, life and financial skills, mentor network development and college/career and post-secondary explorations and planning. The mentor network support team will communicate with families and school personnel to share details of engagement sessions and gather feedback for program improvement and targeting of engagement sessions.
Program Components
Engagement Sessions
The program includes engagement sessions convened virtually in October, November, December, January, February, March and April. These will focus first on social/emotional connections and relationship building with mentors, within the mentoring network and through community connections. The next four sessions will focus specifically on transition goals, plans and explorations and will include shared content and targeted opportunities for students at various stages in their transition plan. These four sessions will include a focus on goal setting, explorations of post-secondary options and life skill needs, workforce participation and opportunities and management of the transition process. Across all sessions a focus on intersectional identity concerns, responsive mentorship and comprehensive network development will be included.
